📑 Paper of the Month

We are pleased to share an interesting paper, “Cerebral blood flow, autoregulation and vascular reactivity in normal pressure hydrocephalus: a systematic review” authored by:

This systematic review pulls together more than 20 years of research on cerebral blood flow (CBF) in NPH.

They found global CBF reduced by about 15% in NPH, and a reduced regional CBF especially in the periventricular white matter, deep grey matter (thalamus, caudate), and frontal lobe, patterns that plausibly relate to gait and executive dysfunction.

But CBF cannot reliably distinguishes patients who will improve after shunting from those who won’t.

What does show promise is cerebrovascular reactivity (CVR). Impaired reactivity before surgery was linked to better outcomes, and it consistently improved after shunting.
