We are pleased to present this month’s interesting paper,
“Redefining treatment expectations: exploring mid- and long-term outcomes of venous sinus stenting in idiopathic intracranial hypertension”
Authored by: Jackson P Midtlien, Carol Kittel, Lucas A Klever, Nicholas R Kiritsis, Jennifer Bernhardt Aldridge, Kyle M Fargen.

This retrospective study assessed long-term outcomes of venous sinus stenting (VSS) in 178 adults with medically-refractory idiopathic intracranial hypertension (IIH) and ≥3 months follow-up. Patients were grouped by symptom trajectory: sustained improvement (30%), temporary improvement with relapse (57%), and no improvement (13%). Most underwent transverse sinus stenting.

Although symptom recurrence was common, VSS consistently reduced intracranial pressure, with 75% showing papilledema improvement and only 17% requiring further surgery. Findings highlight VSS as a pressure-lowering intervention with frequent symptom relapse, informing long-term management expectations.

We are pleased to present this month’s interesting study, “Quantitative brain stem assessment in discriminating neurodegenerative disorders from normal pressure hydrocephalus” authored by:

The study “Quantitative Brain Stem Assessment in Discriminating Neurodegenerative Disorders from Normal Pressure Hydrocephalus” tackles the complex challenge of distinguishing idiopathic normal pressure hydrocephalus (iNPH) from neurodegenerative disorders like progressive supranuclear palsy (PSP), Multiple System Atrophy-parkinsonian type (MSA-P), and vascular dementia (VaD). By employing quantitative metrics such as the midbrain to pons ratio and midbrain area, this research presents a promising avenue for enhancing diagnostic accuracy despite the difficulties posed by overlapping clinical and neuroimaging features. The findings reveal that these simpler indices offer high specificity in differentiating PSP from iNPH, thereby highlighting their potential as valuable tools in clinical practice. This innovative work not only deepens our understanding of neurological disorders but also encourages the pursuit of more effective, streamlined diagnostic strategies, ultimately aiming to improve patient care and outcomes in the field of neurology.

Hydrocephalus is a neurological disorder with an incidence of 80–125 per 100,000 births in the United States. The most common treatment, ventricular shunting, has a failure rate of up to 85% within 10 years of placement. The authors aimed to analyze the association between ventricular catheter (VC) tissue obstructions and shunt malfunction for each hydrocephalus etiology.
