We are pleased to present this month’s interesting paper, “A Randomized Trial of Shunting for Idiopathic Normal-Pressure Hydrocephalus” authored by:
Mark G. Luciano, Michael A. Williams, Mark G. Hamilton, Heather L. Katzen, Nickolas A. Dasher, Abhay Moghekar, Jun Hua., Jan Malm, Anders Eklund, Naomi Alpert Abel, Ahmed M. Raslan, Benjamin D. Elder, Jesse J. Savage, Daniel L. Barrow, Kiarash Shahlaie, Hailey Jensen, M.Stat., Thomas J. Zwimpfer, Jessica Wollett, Daniel F. Hanley, and Richard Holubkov.
It refers to a double-blind, randomized, placebo-controlled trial involving participants selected for shunt surgery on the basis of gait-velocity improvement with cerebrospinal fluid (CSF) drainage.
Among participants with idiopathic normal-pressure hydrocephalus who had a response to temporary CSF drainage, shunting resulted in significant improvements at 3 months in gait velocity and a measure of gait and balance but not in measures of cognition or incontinence.

Idiopathic normal-pressure hydrocephalus is a neurologic disorder characterized by impaired gait, balance, cognition, and bladder control in older adults. The disorder is treated with shunt surgery, but the effectiveness of shunting is unclear.

We are pleased to present this month’s interesting paper, “Redefining treatment expectations: exploring mid- and long-term outcomes of venous sinus stenting in idiopathic intracranial hypertension”
Authored by: Jackson P Midtlien, Carol Kittel, Lucas A Klever, Nicholas R Kiritsis, Jennifer Bernhardt Aldridge, Kyle M Fargen.
This retrospective study assessed long-term outcomes of venous sinus stenting (VSS) in 178 adults with medically-refractory idiopathic intracranial hypertension (IIH) and ≥3 months follow-up. Patients were grouped by symptom trajectory: sustained improvement (30%), temporary improvement with relapse (57%), and no improvement (13%). Most underwent transverse sinus stenting.
Although symptom recurrence was common, VSS consistently reduced intracranial pressure, with 75% showing papilledema improvement and only 17% requiring further surgery. Findings highlight VSS as a pressure-lowering intervention with frequent symptom relapse, informing long-term management expectations.
